AUTHORIZATION FOR ADDITIONAL PAYMENT

Use this form to authorize a one-time withdrawal from your bank account for amounts outside your regular monthly condominium fees — such as arrears, special levies, fob purchases, and similar charges. A Pre-Authorized Debit (PAD) agreement must already be in place before submitting this form.
